Winter Adventure is for teens ages 12-17. To attend Winter adventure you must be 12 years old by January 1, 2025. If you are over the age of 18 and would like to participate in Winter Adventure as a volunteer, please email us at office@cefofmontana.com

Transportation

If needed, CEF of Montana is willing to provide transportation for your teen to the camp. We will contact you with further instructions as the event gets closer.

CEF® CONSENT AND RELEASE FORM

I, the undersigned parent(s) or guardians(s), hereby consent to my teen participating in the CEF summer ministry of Christian Youth in Action®.

Transportation

I give my permission for my teen to be transported to and from this and any corresponding event by the CEF staff or volunteers.

Medical

If my teen has medical conditions which may be relevant to a physician in the event of an emergency, I have listed them on the medical information sheet. In the event that an emergency occurs, I may be reached at the telephone numbers listed below. If I cannot be reached within a reasonable period of time, as determined by the CEF sponsors, I hereby authorize the sponsors to make emergency medical decisions for my teen. If there are any activities, I do not want my teen to be involved in I have listed them below.

Minors Policy

Child Evangelism Fellowship (CEF®) USA Child Protection Policy states, “Even when ministry to children is not taking place, an additional adult or minor must be present when two workers are together and one is a minor, unless the minor’s parent or guardian has signed a waiver.”
I understand that CEF of Montana will contact the parents/guardians if a situation arises where the requirements of the minor’s policy cannot be met. CEF of Montana will seek a resolution that is in the best interest for the minor involved and abide by the parent’s decision.

Audio, Photo and Video

My teen is under the age of 18 and I give permission for their voice, image and/or video to be used for promotional and publicity purposes for CEF of Montana.

Risks

I understand and agree to assume all of the risks which may be encountered during these events and irrevocably and unconditionally release and discharge Child Evangelism Fellowship and its agents, employees, and volunteers from any and all liability, actions, causes of actions, claims, expenses, obligations and damages of any nature whatsoever, which I now have or which may arise in the future, in connection with the described activity or associated activities, including, but not limited to, any injury to my child.
